Q: What is an
Eligibility Packet?
A: It is a 4-page form that has been redesigned into an 11x17 cardstock form printed front and back so it looks like a “folder”
Q: When do I
need to turn an EP in?
A: This form needs to be turned in or renewed every year before you even practice your desired activity, sport or leadership function.
Q: I plan on
being on the chess team; do I need to bother with an EP?
A: Yes, any activity you do with the school needs to have a current schools year’s eligibility packet but only sports activities need a physical.
Q: How often
do I need a physical?
A: It expires every 13 months. If you aren’t sure of the date, we have a copy of your physical in the activities office.
Q: My physical expires in August and football practice starts in June so do I need to renew physical?
A: You need to fill out a current year EP, and since there aren’t any summer dues to pay, you can practice until your physical expires. You must have a current physical to continue.
A: Summer clearance is an opportunity to have an excess amount of helpers go over your EP, verify all signatures and ensure your physical is up to date. If you have any fines, they need to be paid at that time as well. (note: your ASB card and Athletic fee are due at Fall Clearance in August) There are some fall sport practices that require a 2007-2008 EP and current physical. If you are doing a camp sponsored by i.e., WWU, Bellingham Parks & Rec or others, they have their own process.
A: Yes, in the fall is “Fall Clearance” and at that time you pay, ASB card, athletic fees, and any outstanding fines you have. A small piece of paper is filled out by ASB and Athletics for you to give to your coach/advisor and it verifies that you are ready to go!
Q: This year’s summer (2007) clearance is
May 24th; our “big physical update” session at St. Josephs is May 29th. When should I turn in my EP?
A: You can turn it in on the 24th, and the renewed physical will be given to the athletic office to file. If your physical is current and doesn’t expire until the next school year, you will be cleared to do summer sports. Or you can wait until you have the update and give the athletics office the EP then.
